The medical record is the official document of the hospitalization. It is essential as it comprehensively summarizes the patient’s entire care pathway within the facility. It includes:
Medical history and clinical findings.
Diagnostic procedures performed.
Therapy administered during the stay.
A request for a copy of the medical record can be submitted in two ways:
In person: By completing the appropriate form at the Admissions – Secretariat Office (please check opening days and hours).
By email: By sending a written request, in compliance with self-certification laws (please indicate the address to which the documentation should be sent).
The copy can be collected by:
The patient directly.
Another person provided with a written authorization (proxy).
A legally entitled person (for example: a parent exercising parental authority, a legal guardian, a court-appointed administrator, or the attending physician).
All requested documentation will be made available to the patient within the statutory timeframes. The same timeframe applies to requests for reports of diagnostic tests performed during hospitalization, or for any copies requested at a later date.
