There is no predefined form required. The application may be submitted in free form, mandatorily indicating the personal details, contact information of the applicant, and the subject of the request. Documentation to be attached:

  • Copy of a valid identity document (not necessary if the application is digitally signed or sent via personal certified email (PEC)/SPID).

  • Any technical or administrative documentation supporting the request.

Self-certifications: The submission of substitute declarations of certification and affidavits pursuant to Articles 46 and 47 of Presidential Decree 445/2000 is permitted. Currently, there are no online services available for the management or activation of administrative proceedings within the competence of this healthcare facility. Any future activations will be communicated in this section, with the relevant access links.
